Default Dual Display Problem in XP

Hi,

Got an IBM ThinkCentre A30 with an Inno3D MX 4000 PCI using the on board and
PCI to given dual displays.
OS: Windows XP SP1. All this is working fine, until one day after update or
installed some software, the PCI display is not displaying. In the BIOS it
had been set using the onboard first. I had try setting it to use PCI first
and it display on the other display and not the other. So I try reinstalling
the driver for both displays but NO good same result. Got an "error code 10"
under the PCI display card in the Device Manager.

Any idea? No IRQ conflict under device manager! I try also restore with an
Ghost image from before it work, but once I do software or windows updated
then reboot it came back with single display with the same error.
